Student applications to the STIP program require the recommendation of the science resource teacher at your school.  Each science resource teacher may recommend four students per school. If you apply without this recommendation, your application will not be accepted.
All applications must also have two teacher recommendations, also submitted electronically. After you submit the student application, please ask two teachers to return to the site and submit the teacher recommendation form.
All applications and teacher recommendations must be submitted by no later than December 1, 2009.

Complete the application in the following manner:

• Click on the link below to launch the application in PDF format
• Go to file and "save as" STIP Application plus (your name) i.e., STIP Application - Sue Martin
• Fill out the application making sure to complete all information
• Click on the email address at the bottom of the application to send it or save the application to your computer and attach the application to an email addressed to Jill M. Latchana as indicated in the application directions.
