Diploma Order Form [doc][pdf]

A downloadable form to order a diploma for graduating seniors can be accessed here. This form is mandatory for all graduating seniors. Please turn in this form to the counseling office.

Release of Records Form [doc][pdf]

Public law 93-380 requires a parent’s/guardian’s signature (or student’s if 18 years of age) before records can be released. The school is not permitted to release any information without written authority. This form is to be completed by all seniors and turned into the counseling office.

Transcript Request Form [doc][pdf]

The transcript resquest form must be completed before the school can send out official transcripts to colleges. All seniors need to have this form completed and turned into the counseling office.

Summer School

Summer Health Class Registration [doc][pdf]

Students can register to take health courses over the summer.

Foundation Of Technology Registration [doc][pdf]

Students can register to take a Foundation Of Technology course over the summer.

Other Forms

Loss of Credit Contract [doc][pdf]

Students who has lossed credit in any class must fill out this contract and turn it in to the counseling office.
