Important Information:

All new students to MVMS including all 6th graders need to have a current physical form on file in the healthroom (called a Health Inventory Form or HIF) dated this year. Part 1 of this form is filled out by a parent and Part 2 is filled out by doctor or health care provider. See below for a copy of the form or call the healthroom and we can send one home with your child. This form is also required if your child wants to participate in after school sports activities like softball this fall.
