2023-2024 Parent Community Coordinator Assignments (by school)
The My Turn program was developed to assist families with children with developmental disabilities ages 3 – 13. It provides information, education, and referral to help families become connected to services in the community. The program works with families/Specially low-income families and links them to community resources. Services provided: Provides education, resources, and referrals to help families become connected to services in the community. Assistance in connecting families to support systems through the development of natural supports in their communities that lead to circles of support. Connects families to ongoing parent workshops, training programs, and seminars that will help families continue to network as their children become older and their needs change. May provide limited financial assistance for items such as summer camp, recreation, childcare, and supplies e.g. diaper, pediasure. Children currently receiving any other case management services such as Autism Waiver, Community First Choice or Coordination of Community Services (CCS) are not eligible.

Maryland residents who need support with activities of daily living, such as bathing, grooming, dressing, and getting around.
ELIGIBILITY GUIDELINES
Medical and Technical Criteria • Individuals must reside in the community. • Individuals must meet the level of care required to qualify for services in an institution.
Financial Criteria
• Individuals must qualify for Medicaid in the community • Individuals can apply for Medicaid at anytime • Individuals who receive Supplemental Security Income (SSI) are automatically eligible for Medicaid and do not need to apply.
