Please complete all of the forms below and bring to school:

• New Student Information (MCPS Form 560-24)
• Student Emergency Information (MCPS Form 565-1)
• Residency Information (Bring one of the following: Current Tax Bill or Lease from the new home address) 

Enrolling a New Student (From Outside MCPS)

Please complete all of the forms below and bring to school along with the acceptable verification documents:

• Authorization to Request/Release Student Records (MCPS Form 550-2)
• New Student Information (MCPS Form 560-24)
• Student Emergency Information (MCPS Form 565-1)
• Consent and Release Form
• Parent/Child Reunification (PCR) Authorization for Release of Student (MCPS Form 236-1)
• Student Record Card 6 (MCPS Form SR-6)
• Department of Health and Mental Hygiene Immunization Certificate 
• Dental Health Form (MCPS Form 525-17)
• Acceptable Verification Documentation (See image below)

Moving to a New School

• Notify Lake Seneca Main Office - Provide new school name and address for records to be sent directly
