Transcript Requests:
- Authorization to Release Records Form: This form must be completed in addition to the Transcript Request Google Form. A signed hard copy of this form must be submitted to the Registrar's Office before transcripts requests can be completed.

- Mid-Year Transcript Requests: To request a mid-year transcript, please complete this form. Mid-year transcripts will be available to be sent from our registrar's office around the middle of February. Do not use this form for INITIAL transcript requests. Form access will resume in the 2024-2025 academic year.
