This page contains various forms used by the school and the PTA during the school year. In order to save time tracking them down through the appropriate channels, feel free to click on the form you need, download it to your computer, complete it and turn it in to the appropriate party. School Forms
Student Release Authorization Form: Parents and legal guardians may use this form to notify the school of those individuals who are authorized to pick up their child from school.
PTA Forms
Check Authorization Form: Use this form when requesting a reimbursement from the PTA for approved budget items. Please be sure to attach any relevant receipts.
Deposit Form: PTA Chairpersons use this form when submitting money to the Treasurer for deposit in the PTA account.
Teacher Reimbursement Form: This form is used by teachers to request their classroom enrichment reimbursement from the PTA. Please be sure to attach receipts.
